It should illustrate your ability to integrate comprehensive sexual health education into your work, reflecting on the impact and improvements in patient care or education outcomes.

How to Submit

  • Format: Your Practical Application Submission must be compiled into a single PDF document.

  • Email: Once your PDF is ready, submit it via email to admin@evidentiasexualhealth.org. Please use the subject line: "[Your Name] - HealthEquity Toolkit Practical Application Submission".

  • Deadline: Make sure to submit your application after you have completed the HealthEquity Toolkit and paid the relevant membership and certification fees. For renewals, please make sure you are an active member before sending your submission.

Evaluation Process

Your submission will be reviewed by a panel of experts in sexual health education. They will assess the depth of your application, the impact of your initiatives, and the insights gained from your experience. Feedback will be provided, highlighting strengths and areas for improvement.
